Mentre tutto lo staff attende notizie riguardanti il Presidente che è stato ferito e Josh che sta lottando tra la vita e la morte, si scatena la caccia al complice dei due attentatori. Nel frattempo Josh, Sam e Toby ricordano il momento in cui sono stati coinvolti nella corsa alle presidenziali di Jed Bartlet. Leo, Toby e CJ debbono inoltre vedersela con i reporter che attendono notizie sulla salute del Presidente e sulla mancanza di una autorità esecutiva durante l'intervento chirurgico del Presidente. I servizi segreti vengono posti sotto accusa per non aver saputo prevenire l'attentato. Hoynes decide di seguire i consigli di Leo in merito ad una crisi che sta evolvendo in Iraq.
