Brothers Chad and Jared Moldenhauer always had ideas for a retro game and actually built a prototype around 2000, but that proto-game didn’t take, so they went back to their web design and construction jobs, respectively. It wasn’t until the success of the 2010 release, Super Meat Boy, also made by a two-man team, that got the brothers serious again. Chad began working on the animating and Jared on game design. The studio team now includes a developer in Romania, an animator in Brooklyn, and a jazz musician in Ontario.

While the game design was influenced by Disney, and cartoonists Ub Iwerks, Grim Natwick, and Willard Bowsky, particularly their most "subversive and surrealist" elements, game creators Jared and Chad Moldenhauer were mostly inspired by the work of Max Fleischer and Fleischer Studios. Chad has called Fleischer Studios "the magnetic north of his art style."

The actual character of Cuphead was inspired by a 1934 Japanese propaganda film, in which Japanese folklore heroes fight off an army of snakes, alligators, and bat-shaped Mickey Mouse lookalikes. In the cartoon, a teacup-headed man turns into a tank, serving as the foundation for Cuphead’s design.

There were 150 iterations of what would become the Cuphead character, including one with a green kappa with a top hat and one with a fork head.

To stay true to the visual aesthetic of 1930’s cartoons, Studio MDHR had to utilize the same guidelines that were set in that era. Everything is hand-drawn and the backgrounds are painted, even the score was created via analogue means. The one modern technique used was Photoshop, which Chad used to color in the characters.
