29 juillet 1967, au large du Vietnam. L’USS Forrestal, l’un des plus grands porte-avions au monde, est dévoré par les flammes. Pourtant, les Vietnamiens n’y sont pour rien. Ce fleuron de la marine américaine a été frappé par une roquette tirée par l’un de ses propres chasseurs. Neuf bombes explosent sur le pont. Des torrents de carburant enflammé s’écoulent à l’intérieur du navire. Les victimes se comptent par centaines.

Comment une simple défaillance technique a-t-elle pu déclencher l’un des incendies les plus meurtriers de l’histoire de la marine américaine ?

Ce drame, filmé en temps réel par les caméras du bord, révèle bien plus qu’un accident. Il met en lumière les failles d’un système obsédé par la performance, au mépris de la sécurité. Il ne suffit alors que d’une étincelle… pour que tout bascule. Voici le récit d’une catastrophe qui aurait pu être évitée.
