Meredith Grey, the daughter of famous surgeon Ellis Grey, wakes up next to Derek after spending the night with her. She quickly kicks him out as she prepares for the first day of her surgical internship at Seattle Grace Hospital.
There she meets Christina Yang, George O'Malley, Isobel Stephens and Alex Karev. They are supervised by resident Miranda Bailey, nicknamed "The Bully" because of her unfriendly nature. Meredith discovers that Derek is a neurosurgeon at Seattle Grace Hospital.
The trainees then begin their first shift, which lasts 48 hours. A young woman comes to the emergency room with seizures.
Director Derek Shepherd is one of her supervisors. He wants to continue dating Meredith, but she refuses categorically, given their professional relationship.
Meredith learns that her mother, Ellis, has Alzheimer's disease.
