La bataille entre la chirurgie et la médecine interne s'intensifie. Le candidat de Shigekatsu Hiruma, Shinobu Kondo, devient professeur. Takashi Ebina est écarté et Kondo est le principal choix de Hiruma pour le prochain chef de département. Mais un rival surprenant se présente. L'ancien chef de département, Naoki Takano, qui a été transféré à Kanazawa par Hiruma, est rappelé par Kazuyo Mabuchi.
